Indeed, < recalls that Chris Tarrant didn't feel anything was up. However, it seems a lot of the backstage folk, and some of the audience were suspicious.
In other WWTBAM stuff, < looked at some of the more recent stuff which came out after < had moved on. < really didn't like the new clock format, or the new set and lighting.

< haven't watched the documentary, or Who Wants to be a Millionaire, but how could people think of coughing as suspicious? Was it fake sounding and seemed to be intentional?

Question in WWTBAM are done like so:
What they did was have the contestant run through each answer, on the correct answer, a person in the background would cough to let the guy know it was the answer. The cough itself probably wasn't too suspicious in itself, however, the combination of said coughing, and how the contestant would react is what seems to have got them in the end, from what <'s seen.
If you're really curious:
Note that the coughs have been edited to be more clear, according to the documentary, it wasn't as obvious in the original broadcast.
