Nope! ITs basically when someone is born outside the sex binary. TLDR: When someones biologie doesnt match the the anaomty of either male or female. < recomends ^ does some research, as < isnt intersex.

Not always. It could come to realization when puburtiy starts. Yet again < isnt intersex and if ^ would like to know more ^ should do ^'s own research.
