I just double checked and all of those names were changed, so how is one incorrect? Or did I miss one?

Ah, it seems that "The Mamma of All Nightmares" was originally named "Land of the Livid Dead." I think. If so, then I guess Carrot got it.

Confirmation?

That name was never used in Origins. It was first used in Legends as it only had the boss fight. Therefor it's a different name (as it was named Land of the Livid Dead in Origins).
