OK mine is Fairy Glade Phase 1 and 5 and Echoing Caves Phase 2 where you get lead back to The Fairy Glade. Just in case it's version specific answers you want.

Well since I didn't mention a version, I naturally meant the default version mentioned in the first page. In this case, it's the SEGA Dreamcast version.
