I'll take this answer, because it is indubitably correct, even though it is not what I meant. I simply forgot about these! My brain just does not process them the same way as normal levels. My bad.

There are two more levels that fit the description, and whoever names them will get a bonus point. Even though the previous one is unanswered still, I couldn't find anything in the rules which directly forbids having two bonus questions active at the same time.
