I can pronounce in my head but I cannot do it while speaking, no idea why, probably because of my accent it ruins it or because of a minor lisp I had in the past.

Well speaking is a whole different thing then reading or writing. My mother was fluent in English but it took her a year or so to actually have a conversation without messing up.

Strangely enough I've always read English with a British accent when my actual accent is much more leaning to an American one.
