Anyways, what matyuv told me was, unless he creates fake documents from the hospital (by paying them a huge cash amount) he won't have to join, but then comes the problems, if they get cauht, they're in double trouble, or if the hospital refuses they'll be reported for bribary.

Matyuv should still be greatful in a way that he's living in that state, it could be a lot worse.

You shouldn't only think about being lucky to live in a place where it's better than in some other places, but also about the challenges you're up to in your life to your own standards.
