I believe all that does is create a text file to contain the image's file name and code. It doesn't contain the smilies themselves. Those would still have to be downloaded and then uploaded manually.

PHPBB's documentation seem to sound like the images are separate from the pak file itself:

Upload the smilies files themselves and the pack file into the /images/smilies/ directory.

Oh, sorry for the wrong info. I actually can read the images by going to the posting screen and opening the images in a new tab.
