Well, there's the lava pit thing and I guess the other way to kill one is just deplete its Energy bar. I wouldn't know though since I've yet to encounter one in Revolution.

Shoot the Generator until it explodes and the Ninja Henchman goes, and according to Holy Crap, make it fall into the lava.

You can either lure him to the lava pits or just destroying the generator, that's correct.

Quite many. Clearleaf Forest, Prison Ship (and Crow's Nest), Bayou, Walks of Stuff, Sanctuary of Water and Ice, Beneath Sanctuary of R&L, Menhir Hills, Top of the World and Iron Mountains.

The Walks are debatable, but the Crow’s Nest is definitely a distinct level.

Yes, but I meant the Crow's Nest is a part of the Prison Ship itelf. Not the level, the flying ship thingy.

The question specified ‘levels’, so the distinction is kind of important.

Well, as an answer i write this: Woods of Light or Clearing, Bayou, Walk of Life, Sanctuary of Water and Ice, Menhir Hills, Top of the World, Beneath the Sanctuary of Rock and Lava, Walk of Power, Iron Mountains, Prison Ship, Crow's nest. These are in the original, but aren't in GBC version.
