2D Madness? Isn't it the first minigame that's unlocked when you reach some measly number of points? Like 1,000? Is it different on GameCube, or do you mean 2D Nightmare?

Very close, but there's a specific thing which must be done on that version in order for it to unlock the minigame when connected. I'll just accept your answer if nobody else gives the complete one in a while, if that's alright.

I think you need to connect a Game boy advance with a cartridge of Rayman 3 with all the cages unlocked maybe?

Let's see.
The Sanctuary of Stone and Fire has one at the beginning and one at the wrong temple, behind one of the three doors.
One is at the Precipice, second part, while climbing the circular structure.
One is at the Top of the World, second part, in the Teensy throne room (nearly forgot about that one!)
Then there is one in the first part of the Sanctuary of Rock and Lava, before the entrance to the temple proper.
One at the Iron Mountains, first part, after entering the mountain.
And of course the one at the Prison Ship, in the great lava chamber where you fight the ninja pirate.
Also, Ly gives you this after you complete the Walk of Power, and some of the bonus level fairies give you those as well, but I never bothered to try to remember which ones.
I hope that's all of them.

Off the top of my head:
- Smaller viewable area, which makes the game harder. As a consequence, which leads to the following three changes:
- Number of hitpoints increased from 3 (5 with a power-up) to 4 (6).
- Tings not reset after death.
- Various levels were changed slightly to make them easier - some traps removed, power-ups added. The most meaningful to me was the big power-up at the second stage of the final boss battle - it always was so annoying on the PC version that if you die, you have to do the whole thing with only 3 hit points.
- You can replay Mr. Dark's Dare.
- Some music tracks are different (they are also of considerably lower quality, but that's just due to the system's technical limitations).
