The last green gem is in TOTL, Part 4.
When you fought Reflux's second form, there comes the third form. You are standing in the area, where you have to fly to the top. But under this area you found another floor. On an specific place you can jump down and carefully fly to the floor. When you go inside, you find the last gem.

It's at the top of the Tower of the Leptys, when you have to fight the third evolution of the final boss. Let yourself fall off the ledge and you will find a secret room underneath the battle ground, with the last green gem.
